Primary Care Mental Health Support Service

The Primary Care Mental Health Support Service works with Croydon residents who have achieved some stability in their recovery from mental illness and are motivated to increase self-management and independence. The service brings together a team of health and social care professionals, including nurses, social workers, support workers, and psychiatrists. SLaM maintains close links with colleagues in the voluntary sector and primary care.

The Primary Care Team actively involves the service user and carer in identifying the needs and strengths of the service user. They work closely with them to build on these strengths and access other resources to help meet their needs and reduce dependence; and to promote the service user’s well-being, quality of life and progress through the mental health care pathways.
